Town Clinician / Referral Form

The Town of Millis contracted services through Community Impact, Inc. to co-response with the Millis Police Department, assist with resources and referrals to the Public Schools and Elder Services, and provide health education to town residents of all ages. The safety of our community is our highest priority, and to have Community Impact, Inc. as a resource within the town adds another extremely important dimension to how Millis Police can help residents in the community.

Services Provided by Community Impact Inc.

Provide comprehensive professional social work services to assist Millis residents with social, emotional, and economic needs. Services include transitional case management, consultation, information and referral, advocacy and support, and crisis intervention. All services are confidential and open to town residents of all ages and incomes.

We accept referrals from various sources, including community residents, schools, the police and fire departments, clergy, doctors, hospitals, and mental health agencies. We collaborate with other community agencies and meet with residents at the Millis Police Department, Schools, Town Offices, Community Impact, or at their residence with a Millis Police Officer.
